Competitive Player Forms for the 2008-2009 Season

Kingwood Alliance Soccer Club

Fall 2008 - Spring 2009 Competitive player fees will be the following:

$175.00 registration fee

•      Competitive Player Registration Form

•       Medical Release Form

•       Competitive Scholarship form

Please print both forms, fill out and send in along with a copy of the players

birth certificate to the soccer office.  Per STYSA Insurance regulations, each player

must fill out the forms along with payment in order to tryout.

Disregard the word "team" on the mailing address line (in second Section of form)

The "school" and "grade" blanks in the third section of the form
